The Cedar Foundation is seeking to recruit the following staff members:
Transition Officer REF: 18/013/TO/WEB
The Transition Officer will use an inter-agency approach to ensure a smooth transition post school into education, employment as well as exploring health & well-being opportunities in their local area.
Hours: 12 Hours per Week
Contract: Temporary; to the end of December 2018 (Initially)
Location: Belfast Health & Social Care Trust
Hourly Rate: 3 point incremental scale starting on £21,268 per annum
Essential:
- QCF level 5/ Degree or equivalent level in a relevant discipline such as Health & Social Care
- Minimum of 1 years paid’ experience in the direct provision of education, training or support
OR
- QCF 3 or equivalent in a relevant discipline`
- Minimum of 3 years’ paid experience as above
- Experience of Supporting people
- Experience of building positive relationships with a wide variety of stakeholders
- Proficiency in IT skills specifically Microsoft Word, Excel and Outlook
- Ability to work independently with good organisational, communication & interpersonal skills
- Hold a full current driving license valid for use in the UK and Ireland and have access to a car on appointment.
This criterion will be waived in the case of a suitable applicant who has a disability which prohibits them from driving but who is able to organise suitable alternative arrangements in order to meet the requirements of the post in full.
Desirable:
- Experience and understanding of the barriers in relation to disability
- Experience of Transitional Planning
- Facilitation and group work skills
- Previous experience of interagency working
