Key Responsibilities:
Conduct regular client support plan reviews, completing and updating individual support plans; within contractual timeframes and work with clients to achieve their identified goals in accordance with The Salvation Army policy.
Work with colleagues to design and deliver a range of in-house support provisions designed to achieve successful outcomes.
Complete and review client’s assessments including risk assessments, within contractual timeframes, using a person-centred and trauma sensitive approach.
Support successful transitions for prison leavers back into the community.
The successful candidate(s) will be able to demonstrate:
- Knowledge of the main support needs which are commonly faced by people experiencing homelessness.
- Ability to work in a busy environment and can demonstrate ability to make decisions under pressure.
- Knowledge of the benefits system and an understanding local housing legislation and how to advocate and maximise housing points.
- Ability to use your initiative in problem solving for the benefit of clients and colleagues.
- Excellent verbal and written communication skills.
- Experience in harm reduction and support people in active addiction.
- Ability to work alongside the Criminal justice department.
- NISCC registration is also required for the post.
