Job Summary: The Senior Care Assistant (SCA) is responsible for providing high-quality care to residents by assessing their needs, implementing personalised care plans, and continuously evaluating and updating these plans to ensure the best possible outcomes. This role involves a combination of direct care responsibilities, medication management, and key worker duties for a small group of residents. The Senior Care Assistant is also responsible for maintaining accurate records, participating in multidisciplinary team meetings, and ensuring effective communication with residents, their families, and other staff members.
Key Duties and Responsibilities:
- Care Assessment and Planning:
- Assess residents' care needs and develop individualised care plans.
- Continuously evaluate and update care plans to reflect any changes in residents' needs.
- Documentation and Record Keeping:
- Complete all relevant paperwork, including progress notes and updates on activities of daily living.
- Maintain accurate records of residents’ care, communication, and other pertinent information.
- Medication Management:
- Order, handle, administer, store, and oversee the custody of medications.
- Ensure that all medication procedures are carried out safely and in accordance with regulations.
- Key Worker Responsibilities:
- Act as a key worker for a designated group of residents, providing personalised care and support.
- Build strong, trusting relationships with residents and their families.
- Team Collaboration:
- Attend annual review meetings and contribute to multidisciplinary team discussions to ensure comprehensive care for residents.
- Collaborate effectively with other team members to provide holistic care.
- Reporting and Evaluation:
- Report on residents’ progress, incidents, and any changes in their condition.
- Regularly evaluate residents' records to ensure they are accurate and up-to-date.
- Communication:
- Maintain open and effective communication with residents, their families, and staff members.
- Ensure that all communication is accurately documented.
- Professional Development:
- Engage in ongoing professional development, including mandatory in-house and digital training.
- Stay updated on best practices and new developments in residential care.
- Participate in supervision and appraisals as part of on-going professional development process.
- Staff Coverage:
- Provide cover for off-duty or annual leave for a small group of staff as needed.
- Ensure continuity of care by effectively managing shifts and workloads.
- Policy and Procedure Adherence:
- Adhere to and implement all of the home’s policies and procedures.
- Ensure that care practices are compliant with regulatory standards and organizational guidelines.
