The Connected Community Care service will enhance the quality of life for people, their families and carers from diagnosis through treatment and beyond. The service will support people to ensure they get the right support at the right time enabling selfdirected support and encouraging self-management. This approach will span across health and social care, statutory and voluntary sectors providing a principal point of contact to sources of support and services for everyone who has affected by cancer. The Macmillan Community Link Worker will support people affected by cancer through a meaningful conversation identifying their holistic needs and co-creating a plan to address the identified needs and what matters to them. They will monitor and review the needs of individuals and their family/ carers to ensure the best possible outcomes are achieved. They will assist in the navigation and co-ordination of a wide range of appropriate services and access to support ensuring the individual and their family/ carers needs are met providing a seamless pathway across their cancer journey.
