We are looking for a passionate, dedicated individual to join our team to provide health and wellbeing advice and support to Older People across Ballymena, Larne and Carrickfergus.

The IMPACTAgewell® Officer works within IMPACTAgewell® locality hub teams including GPs, Community Pharmacy and NHSCT to support older people in their health and wellbeing. Visiting older people at home and listening to what matters to them to enable them to 'agewell' at home.

IMPACTAgewell® has grown from working with six GP Practices in 2017 to a partnership of 22 in 2024 with plans for more scale and spread ahead.

Officers provide empathetic and sensitive emotional support to older people with long term health conditions via a personalised health and wellbeing action plan.

Connect them to internal and external services, supporting independence and improving quality of life.

We are looking for  passionate, dedicated individuals to join our team to enable to us to continue to support older people across Mid and East Antrim.
