To identify and address local community health needs and issues.  The post holder will also work as an enabler to develop community skills and increase knowledge in local neighbourhoods with regard to health and well-being issues. The post holder will have a facilitative development role across East Belfast for all four East Belfast Health Framework themes generally but will have specific responsibility for supporting and developing chronic disease prevention, health observances, chronic pain management and pharmacy programmes.

The post holder will work as part of a team to support the achievement and full implementation of the East Belfast Health Framework which contains the following 4 themes:

  • Healthy Minds
  • Healthy Bodies
  • Healthy Relationships
  • Healthy Neighbourhoods.

Role:

To undertake community visits to EBCDA members, community organisations and relevant stakeholders to:

  • Update on health information
  • Raise awareness of available health and well-being programmes or training opportunities
  • Develop and support local health focussed work
  • Identify the health and wellbeing needs within the local communities, especially of vulnerable and high-risk groups living in East Belfast

Increase East Belfast’s connectivity to voluntary organisations who deliver programmes and services supporting the East Belfast Health Framework to:

  • Increase participation locally
  • Co-ordinate programmes and training opportunities
  • Promote opportunities for joint working between community/voluntary/statutory organisations to address chronic disease prevention, pharmacy and health observances

To develop, coordinate, deliver and support programmes, as well as promote opportunities for joint working across sectors to:

  • Improve connections for East Belfast’s health development work.
  • Deliver a Healthy Living Centre Better Days Pain Support Programme and develop existing pain work
  • Develop chronic disease prevention work including a focus on diabetes and blood pressure
  • Maximise health observances initiatives
  • Build pharmacy connections and new initiatives
  • Build GP connections and new initiatives

To build the capacity of the community sector in East Belfast to engage with and influence health policy.

Work together with EBCDA health staff to ensure the ‘Take 5 Steps to Wellbeing’ messages are implemented through all aspects of EBCDA’s health work across East Belfast communities and especially with the Chatty Bench hosts.

To support statutory agencies and health professionals to build stronger relationships with the community.

Represent and host local health development initiatives, projects and programmes in East Belfast that improve health and social wellbeing as well as local and regional inter-agency fora and groupings.

Utilise community knowledge to identify gaps in health provision and work with EBCDA health staff to initiate a response, encouraging the full and active involvement of communities in this process.

Liaise closely and develop valuable links with key health and social wellbeing stakeholders across community, voluntary and statutory sectors.

To link with local, citywide and regional networks to facilitate information sharing, networking and learning.

Plan, allocate and manage all resources to ensure strategic objectives are achieved within resource limits.

Promote health initiatives across a variety of media.

Ensure EBCDA intervention recording, monitoring and evaluation systems are implemented, developed and maintained and provide funders and EBCDA Board with relevant reports.

Undertake any other reasonable duties required by EBCDA towards the successful regeneration of the area.
