Working closely with local Respiratory teams, you will make a difference to clients and their families by supporting them in achieving outcomes which will enhance their quality of life and independence.
Working closely with local Respiratory teams, as well as NICHS colleagues, you will make a difference to clients and their families by supporting them in achieving outcomes which will enhance their quality of life and independence.
You will be providing a needs based support service to people affected by chronic lung conditions which will include:
- Developing referral pathways and establishing new services
- Pro-actively maintaining existing networks and partnerships;
- Receiving referrals from a variety of sources
- Carrying out client needs assessments via home visits
- Delivering NICHS Taking Control Programme and Breathing Better Support Groups and networks
