PLEASE RSVP BY THIS THURSAY 13TH OCTOBER! Community and voluntary sector organisations working in the Larne, Carrickfergus and Newtownabbey areas are invited to a networking event with the East Antrim Integrated Care Partnership on Thursday 20th October.
Who we are?
Integrated Care Partnerships (ICPs) are networks of health and social care providers, including GPs, pharmacists, local councils, health trusts, service users, carers, along with the Voluntary and Community sector, which aim to transform how health and social care is delivered. East Antrim ICP covers Larne, Carrickfergus and Newtownabbey.
What is this event about?
We want to involve Community and Voluntary sector organisations from the East Antrim area in the design and delivery of improved health and social care pathways and we need to find out more about what you do.
Why should you come along?
Find out about the progress being made by ICPs in bringing care closer to people’s homes in the areas of stroke, diabetes, respiratory, frail older people and end of life care. You will have the opportunity to explore how your services can be further integrated with health and social care, and to network with other service providers.
