The Cedar Foundation is seeking to recruit the following staff members:

Cedar delivers a range of services that enable people with disabilities to get the most out of life and to be fully included in their communities. Our services are centred around the individual needs of each person and are delivered throughout Northern Ireland.

 

Due to rapid continual growth and development, a fantastic opportunity has arisen for a Transition Officer, to support and assist service users.

 

 

The Transition Officer will deliver the Transition; Let’s Get Ready Service.  The service fully involves young people and their families to make decisions about their life after school.  The Transition Officer will use an inter-agency approach to ensure a smooth transition post school into education, employment as well as exploring health & well-being opportunities in their local area.

 

 

Hours:                                    37 Hours per Week

Contract:                    Temporary; to the end of December 2018

Location:                    Belfast Health & Social Care Trust

Hourly Rate:             3 point incremental scale starting on £21,268 per annum

 

 

Essential:                  

  • QCF level 5/ Degree or equivalent level in a relevant discipline such as Health & Social Care

  • Minimum of 1 years paid’ experience in the direct provision of education, training or support

OR

  • QCF 3 or equivalent in a relevant discipline`

  • Minimum of 3 years’ paid experience as above

     

  • Experience of Supporting people

  • Experience of building positive relationships with a wide variety of stakeholders

  • Proficiency in IT skills specifically  Microsoft Word, Excel and Outlook

  • Ability to work independently with good organisational, communication & interpersonal skills

  • Hold a full current driving license valid for use in the UK and Ireland and have access to a car on appointment.

     

    This criterion will be waived in the case of a suitable applicant who has a disability which prohibits them from driving but who is able to organise suitable alternative arrangements in order to meet the requirements of the post in full.

 

Desirable:                 

  • Experience and understanding of the barriers in relation to disability

  • Experience of Transitional Planning

  • Facilitation and group work skills

  • Previous experience of interagency working
