Practice Team Leader (fixed term maternity cover)

You will be expected to organise and be responsible for the delivery of ‘hands on’ person centred care and support in the location, working within the organisational and professional frameworks; to improve the daily lives of the people we support, through clearly defined, regularly reviewed and carefully implemented support plans that continually enrich their life experience. You must have an NVQ3 in care and management, or QCF equivalent. You must be able and willing to drive, hold a full UK driving licence or equivalent and have access to a car on a daily basis.
This post is fixed term for 9 months.
