The Part Time Transitions Support Worker will work with young people in the Mid and East Antrim area who are hard to reach, isolated and those likely to engage in risk taking activities including anti-social behaviour, substance misuse and young people li

Provide opportunities for young people to engage and develop their capacity to make informed choices about their health through:

Work with Individual Young people to offer:

▪        1 to 1 support through a mentor so that they will cope better with everyday life

▪        Training in the areas of personal development and good relations so that they feel better about themselves

▪        Opportunity to participate in community engagement projects so that they can get involved in their community

Work with mentors to:

▪       Ensure training for Mentors

▪       Provide a direct point of contact

▪       Provision of support & supervision of mentors

Work as part of the Youth team:

▪        To engage and build appropriate relationships with young people

▪        To work with young people to develop community projects

▪        Provide support for young people through RAPP and personal development work

▪        Co-operate with other programme staff across the Association to ensure that every young person has the opportunity to reach their full potential

Work with the Transitions Support Worker to:

▪        Evaluate and report on aspects of programme content and delivery, which could be improved.

▪        Ensure co-ordination of individual support of young people within the association

▪        Deal appropriately with suggestions and complaints

▪        Make recommendations for expenditure

▪        Monitor and control the use of resources

▪        Develop self within the job role

▪        Plan activities and determine work methods to achieve objectives

▪        Ensure the implementation of Association policies through the programme activities i.e. equal opportunities, protection of children and vulnerable adults, health and safety, etc.
