To be responsible for the implementation, delivery and evaluation of assessment and treatment programmes for patients referred to Occupational Therapy from the In-patient Unit and Day Hospice
Principle Objectives
To manage a defined caseload using evidence based, patient centred principles to assess, plan, implement and evaluate clinical interventions.
To expand experience and develop skills and knowledge in palliative care.
Clinical Duties include:
- To plan and organise a designated caseload of patients and own activities.
- To be responsible for the assessment of patients referred from within the prescribed area.
To provide an OT service for palliative care patients using a holistic approach aiming to maintain or improve quality of life through assessment, planning, implementing and evaluation of treatment and rehabilitative programmes of care for patients in Northern Ireland Hospice including the In-patient Unit, Day Hospice and outpatients.
To work within all domains of the patient’s life; to be aware of their physical, psychological, social, spiritual, emotional needs and hopes and to provide appropriate OT intervention within their domains referring to other health care professionals according to identified need.
To organise and carry out home assessments when necessary and ensure close liaison and communication with the Community Occupational Therapy Service to facilitate discharge management and planning.
To attend Multidisciplinary meetings and to ensure that the most effective reporting mechanisms are in place.
