Locality Community Navigator based in Cookstown

The main purpose of the Locality Community Navigator will focus on the promotion and development of preventative and reablement services for older people within the Community/Voluntary sector that focuses on health and wellbeing and the reduction of social isolation.

Main Responsibilities

-        To understand and utilise the concepts of Asset based Community Development, the promotion of Health and Wellbeing, Prevention, Reablement and co-production of new supports and systems with Locality partnership steering groups;

-        To actively support the implementation of social support model by engaging with the Health and Social Care team including GPs, other stakeholders and building and sustaining positive working relationships;

-        To act as a local link between the NHSCT and Community/ Voluntary sector groups within the locality;

-        To support the development of appropriate signposting to the Community/Voluntary sector;

-        To identify gaps within the locality and support the Community/ Voluntary sector to secure sustainable funding to meet gaps in provision and to build capacity;

-        To support local Community/Voluntary groups with governance arrangements and the development of policies and procedures;

-        To asset map in this locality of the Northern Trust to support and maintain the Trust’s Web based directory ensuring that information is made available to the public and relevant acute and community teams;

-        To support the Partnership Steering Group in each locality and facilitate meetings as required;

-        To educate relevant acute and community teams regarding services within the Community/Voluntary sector and referral processes;

-        Work with the Partnership Steering Group to identify opportunities for the recruitment of volunteers and to signpost to relevant sources of training;

-        To hold events that engage all stakeholders on themes which promote wellbeing as required;

-        To promote and develop engagement from all aspects of the community including Statutory Organisations, Councils, Housing Executive, PSNI, Business Community and learning establishments;

-        To link with ICPs regarding Community/Voluntary services to support the development of new care pathways; and

-        To be knowledgeable regarding funding streams and roles of other government departments such as the Department for Social Development.

  • Demonstrate commitment by regular attendance and the efficient completion of all tasks allocated.

Skills and Experience Required:

-        Experience of working with older people in the community

-        Proven experience of  working within a  client centred context

-        Experience of using a computerised recording system

-        Experience in service quality assessment

-   Demonstrate knowledge of assessing the needs of older people

-   Knowledge and experience of audit and research skills, evidence and effectiveness

-        Excellent written and verbal communication skills

-        Ability to work independently/as part of a multi-disciplinary team

-        Ability to achieve objectives and meet agreed deadlines

-        Ability to motivate individuals and act as a champion for services for older people

-        Computer skills

-        Project Management Skills

-          Negotiation skills

-          Post holder will be subject to an enhanced Access NI check

-          Full driving licence and access to a car for business purposes or access to a form of transport that will meet the travel requirements of the post

-          Must be able to work evenings and weekends if required.

Qualifications

-        Level 5 in Health & Social Care Management AND 2 years experience working with older people in the community

OR

-        HNC in Health and Social care AND 3 years experience working with older people in the community

OR

-        5 years experience working with older people in the community.

What we can offer you:

Salary Scale £21,069 - £22,178

Hours: 35 hours Monday – Friday 9am – 5pm, with flexible working policy.

Fixed Term Contract up to 30 June 2019 (with possible extension)

Annual Leave: 26 days plus 12 Bank Holidays
