IMPACT Project Officer (Full Time & Part Time)

These posts will have shared responsibility for co-ordinating the development and delivery of personalised health and wellbeing plans for older people living with long term health conditions in partnership with a local health & wellbeing hub.
Mid & East Antrim Agewell Partnership (MEAAP) are delighted to be the lead partner in an exciting community based healthcare initiative – namely the “Involving Many to Prescribe Alternative Care Together” (IMPACT) Programme funded by the Dunhill Medical Trust.
We are seeking 6 enthusiastic & experienced individuals to support the delivery of this three year project, due to commence 1st April 2017, all of whom will be office based within the Mid & East Antrim Borough Council area. Secondment opportunities will be considered.
· 3 x Full Time IMPACT Officers (35 hours per week)
· 3 x Part Time IMPACT Officers (20 hours per week)
Salary: £20,868 pro rata + 3% Workplace Pension + 25 days Holiday Entitlement
These posts will have shared responsibility for co-ordinating the development and delivery of personalised health and wellbeing plans for older people living with long term health conditions in partnership with a local health & wellbeing hub involving General Practitioners, Community Pharmacists and Northern Health & Social Care Trust representatives.
